Question reports
Grouped by question. Priority = reported by 3+ different
learners, or any "offensive" report. Confirm or Fixed gives every reporter (who reported before the fix) a
small Seeds reward, once per question, within daily/weekly caps; Not a problem and Duplicate give nothing.
Learner identities are never shown. To find what to edit:
ops/report_source.py <question id>.
